A medical exam performed on Wednesday showed the Portugal winger's right ankle had improved but not enough to return to action in the next week.
Madrid will play a Spanish league match against Getafe on Saturday. The team's website said that Ronaldo would undergo another medical test next week, with Madrid playing at the San Siro on Tuesday.
Madrid has lost three games without Ronaldo, including Tuesday's Copa del Rey match against third-division club Alcorcon. Ronaldo sprained his ankle in a 3-0 win over Marseille last month, and the injury recurred while he was away on World Cup qualifying duty soon after.
Both Madrid and Milan have six points in Group C with three games to play.
