The often-injured Boozer hurt his ankle during Utah's 97-71 victory over the Denver Nuggets on Friday night, according to Jazz coach Jerry Sloan.
Boozer missed 44 games with a strained quadriceps tendon, and he also had knee surgery.
Boozer, who is averaging 16.9 points and 10.9 rebounds in 18 games, returned to the lineup on Feb. 23.
Paul Millsap replaced Boozer in the starting lineup for Sunday's game in Toronto.
