(CBC) An RCMP officer in Burnaby, B.C., has been charged with two counts of dangerous driving causing death in connection with a crash at Kingsway and Royal Oak Avenue last October.
Const. Petina Kostiuk has been charged in connection with the deaths of Albert Haczewski, 27, and Koyo Walter Hara, 26. Both men died of their injuries after a police cruiser slammed into the car they were in during the morning hours on Oct. 31.
In addition to the criminal charges, Burnaby RCMP are conducting an investigation into Kostiuk's conduct and have requested the Vancouver Police Department to review their investigation, RCMP spokeswoman Const. Alexandra Mulvihill said Monday.
Police have issued a summons for Kostiuk — who has been off duty since the crash — to appear in court at a later date. She's traumatized and continues to recover from the injuries she suffered in the collision, Mulvihill said in a news release.
Several members of the RCMP, including Kostiuk, responded to a report of a suicidal woman around 1:50 a.m. on the night of the crash.
Kostiuk's cruiser, which was headed west on Kingsway, went through a red light at Royal Oak Avenue. Her Cruiser collided with a grey Chevrolet Cavalier that Haczewski was driving south on Royal Oak Avenue. Hara was his passenger.
The impact spun the police car and the Cavalier around and they crashed into an SUV that was stopped on Kingsway.
The two men in the Cavalier were trapped and rescue crews used the Jaws of Life to cut into the passenger side. They later died in hospital.
Kostiuk and the driver of the SUV were taken to hospital with non-life-threatening injuries.
