HMCS Chicoutimi (Canadian Press)
The inquiry into the fire on HMCS Chicoutimi made a number of recommendations to prevent such fires in the future. Here are some of the highlights:
- The Canadian navy should install "splash-proof" insulation on electrical wires to prevent further electrical fires caused by salt water.
- The navy should study the performance of the Victoria-class submarines operating on the surface in various weather conditions and at various speeds.
- Standard operating procedures for conning tower operations should be developed and enforced, specifically for when the tower's hatches should be opened when the sub is sailing on the surface.
- Procedures for the use of emergency breathing equipment should be updated so that the masks are more readily available and visible in low-light conditions.

"I accept, without reservation, the board's findings that the decisions made by Cmdr. Luc Pelletier were reasonable given the information available to him, both before and after the fire … I am confident the recommendations … will result in a safer environment for submariners."

"As commanding officer of HMCS Chicoutimi, I consider myself privileged to command and serve with this crew of which I am very proud. I have also always understood that this privilege comes with great responsibility. I spoke with Vice-Admiral MacLean yesterday morning and clearly understand his assessment and fully accept his decision, as I have always been prepared to do."
