Health Canada says the Geometric Stacker wooden toy has more barium in the paint than allowed.
Some forms of barium can cause nausea, vomiting, diarrhea and cramps as well as more severe symptoms.
There are no reports of illness.
About 300 of the toys, 25 pieces painted in red, yellow, blue, green, purple and orange with the item number 094730, have a universal product code 000772005678 and batch number ZP1207 on the base.
Consumers can return the toy to Toys "R" Us for a full refund.
For more information, consumers may call Toys "R" Us at (800) 869-7787, 24 hours a day, seven days a week.
